POSITION OVERVIEW
Harbor Health is seeking Care Coordinators (LVN) to join our clinic operations team in Austin, TX. Harbor Health is an entirely new multi-specialty clinic group in Austin, TX utilizing a modern approach to co-create health with those who get, give, and pay for it — allowing everyone to fully flourish. In this pivotal role, you will apply your LVN clinical judgment and Harbor Health-approved protocols to efficiently manage and organize incoming clinical communications, ensure timely responses, and facilitate effective communication between providers and members. You will work closely with our clinic care team to manage provider clinical inboxes, support medication refill and care plan coordination, and address member questions and concerns — bringing strong organizational skills, attention to detail, and a thorough understanding of clinical terminology to everything you do, in alignment with our mission of transforming healthcare through our payvider model.
POSITION DUTIES & RESPONSIBILITIES
Clinical Inbox & Communication Management
Monitor and prioritize provider incoming clinical communications, including member cases and portal messages, based on urgency, member priority, and clinical relevance
Route communications to the appropriate clinician or clinic staff for timely review and response; escalate urgent messages to the appropriate personnel for immediate attention
Serve as a liaison between providers, members, and other stakeholders; facilitate communication between clinicians regarding care plans, test results, and treatment updates
Maintain accurate records of all incoming and outgoing communications; document member inquiries, responses, and follow-up actions in the EHR or other designated systems
Review outgoing communications for accuracy, completeness, and adherence to clinical guidelines
LVN Clinical Support
Apply LVN clinical judgment and Harbor Health-approved protocols when reviewing symptom-related messages, medication refill requests, and care plan questions submitted through the clinical inbox, escalating anything outside LVN scope to the appropriate RN, NP/PA, or physician
Support medication refill review and routing in accordance with approved protocols and provider orders
Provide member education on medications, care plans, and follow-up instructions within LVN scope of practice
Monitor incoming messages for red-flag symptoms or safety concerns and escalate immediately per Harbor Health protocol
Support chronic disease management and preventive care follow-up coordination as directed by the care team
Process Improvement & Team Support
Identify opportunities for process improvement and efficiency enhancement in clinical communication workflows
Collaborate with healthcare professionals to develop and implement best practices for inbox management
Provide training and support to clinical staff on effective inbox management techniques and tools
Offer guidance on navigating electronic communication platforms and EHR systems
Address inquiries and troubleshoot issues related to inbox management as needed
Participate in special initiatives at the direction of market leaders
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
Active Texas LVN license or compact state privilege
3+ years of experience in a medical office or healthcare setting
2+ years of clinical experience as an LVN in ambulatory care, primary care, or a similar outpatient setting
Strong organizational skills, attention to detail, and a thorough understanding of clinical terminology and procedures
Comfort with technology, including the Microsoft suite of products
Prior experience using electronic health records, including data capture, data mining, and reporting
Ability to manage multiple tasks simultaneously and maintain an organized, systematic workflow
Flexibility in an ambiguous and dynamic environment with a positive, solutions-oriented attitude
Demonstrated ability to engage effectively and with cultural sensitivity across diverse populations
Strong problem-solving skills and sound judgment for identifying and escalating issues as needed
Low ego and humility; ability to build trust through clear, consistent communication
Preferred: Experience with Athena or similar EHR and clinical messaging platforms
Preferred: Bilingual in English/Spanish
