POSITION OVERVIEW
We are seeking a PRN Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our innovative healthcare team at Harbor Health. In this role, you will provide high-quality, member-centered care on an as-needed basis, supporting our mission to transform healthcare delivery in Austin. This position offers flexibility while maintaining our commitment to comprehensive, collaborative care.
POSITION DUTIES & RESPONSIBILITIES
- Deliver comprehensive primary care services to members on an as-needed basis
- Perform thorough health assessments, diagnose conditions, and develop treatment plans
- Prescribe medications and treatments within scope of practice
- Document all encounters accurately and thoroughly in the electronic health record
- Collaborate with primary care providers and specialists to ensure continuity of care
- Participate in care team meetings when scheduled for shifts
- Respond to member health concerns and provide appropriate follow-up care
- Support care management initiatives and population health programs
- Maintain compliance with all clinical protocols and organizational policies
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
- Current Texas state licensure as a Nurse Practitioner or Physician Assistant
- DEA registration and prescription authority
- Willing to travel between Harbor Health clinic locations across Greater Austin
- Minimum of 2 years of clinical experience in primary care or relevant specialty
- Experience with value-based care delivery models preferred
- Strong understanding of preventive care and chronic disease management
- Excellent communication and interpersonal skills
- Proficiency with electronic health records
- Ability to work independently while maintaining collaborative relationships
- Flexibility to work various shifts as needed
