About The Role
Are you ready to be a part of the cutting-edge healthcare workforce at Sprinter Health? As a member of our healthcare team, you will play a pivotal role as a full-time patient advocate in the role of a Health Navigator. This position is dedicated to improving patient care and outcomes by providing crucial health education, delivering and collecting diagnostic medical kits, gathering health data, and ensuring seamless drop-offs of essential supplies. Your mission will be to empower patients with the knowledge and resources they need to manage their health effectively while contributing to the overall success of our healthcare organization.

A day in the life of a Health Navigator …
Commencing the day… Your day will begin by ensuring that you have all of the proper supplies and PPE with you to care for that day’s patients
Navigating with ease… Using easy and modern technology, you will access your schedule for the day and commute to each patient’s home
Creating meaningful bonds… Once you have arrived, you will have the opportunity to make a warm and welcoming connection with a diverse range of patients
Patient-centric, education and collection… During your appointments you will guide patients through health questionnaires and provide patients with comprehensive instructions on how to use various types of Fit Kits. You'll need to communicate complex medical and social information in a clear and empathetic manner, ensuring that patients understand the importance of following prescribed protocols.
Kit collection and management… You will ensure all kits are labeled properly and then coordinate drop off to the appropriate lab or shipment facility
Strong communication skills (both verbal and written) with the ability to convey medical information clearly and empathetically in both English and SpanishExcellent organizational skills to manage kit collections and drop-offs effectivelyCompassionate and patient-centered approach to healthcare advocacyConsistently exhibit the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health servicesTechnologically savvy and comfortable using tools such as mobile devices for charting and HIPAA secure messaging apps for care coordinationAbility to work independently with minimal supervisionReliable transportation as you will be using your own vehicle to travel to patient homesAbility and willingness to drive for significant portions of the dayAbility to physically access and navigate patient homes including but not limited to climbing stairs and carrying up to 30 pounds while walkingPass national background check, DMV check, Knowledge of healthcare protocols and medical terminology (preferred)

Skills and Requirements
- Strong communication skills (both verbal and written) with the ability to convey medical information clearly and empathetically in both English and Spanish
- Excellent organizational skills to manage kit collections and drop-offs effectively
- Compassionate and patient-centered approach to healthcare advocacy
- Consistently exhibit the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Technologically savvy and comfortable using tools such as mobile devices for charting and HIPAA secure messaging apps for care coordination
- Ability to work independently with minimal supervisionReliable transportation as you will be using your own vehicle to travel to patient homes
- Ability and willingness to drive for significant portions of the day
- Ability to physically access and navigate patient homes including but not limited to climbing stairs and carrying up to 30 pounds while walking
- Pass national background check, DMV check

To begin working with patients you will need to have …
