They’re growing their team and looking for a Community Health Worker to support outreach, patient engagement, and community-based health initiatives across all five boroughs.

This is a hands-on, impactful role where you’ll:

• Connect directly with individuals managing asthma and chronic conditions
• Conduct outreach and health screenings (in community + some home visits)
• Help coordinate resources like air purifiers and environmental support
• Educate and advocate for healthier living environments

• Spanish bilingual (required)
• Someone self-motivated, organized, and comfortable working independently
• Passion for community health and helping underserved populations
• Ability to connect with people and build trust across diverse communities
