About the role
Social Worker educates members on improving health outcomes, assesses Social Determinants of Health (SDOH) and community resource needs, participates in process improvement and other pilot programs as they arise, and works with other Oscar teams to ensure care for our members
You will report to the Nurse Manager of Case Management.

If you live within commutable distance to our New York City office (in Hudson Square), our Tempe office (off the 101 at University Dr), or our Los Angeles office (in Marina Del Rey), you will be expected to come into the office at least two days each week. Otherwise, this is a remote / work-from-home role.

Pay Transparency:
The base pay for this role in all locations is: $27.31 - $35.84 per hour. You are also eligible for employee benefits and monthly vacation accrual at a rate of 10 days per year.
Responsibilities
- Help coordinate care across a variety of settings (inpatient, outpatient, post acute, ER, home care) while maintaining strict confidentiality
- Reach out to members undergoing difficult health challenges, partner with them to develop unique care plans, and serve as their guide as they navigate the healthcare system, with the ultimate goal of creating an excellent experience.
- Assess and support SDOH needs of our members, and educate members on resources for community support programs.
- Participate in crisis intervention for members.
- Be a member advocate and address any barriers to care or compliance with the care plan.
- Communicate with members by phone or secure messaging to provide education on health conditions, new medications, and procedures.
- Compliance with all applicable laws and regulations
- Other duties as assigned
Qualifications
- 2+ years of Medical social work experience in a healthcare facility setting. (Ex. Hospital, SNF, Home Health, Hospice)
- 1+ years experience Care Coordination and Navigation
- Masters in Social Work (MSW) from a graduate of a Nationally Accredited School approved by the Council on Social Work Education.

- Bilingual in Spanish and English
