Today, 60 million Americans living in rural communities are facing a crisis of access to care. In the U.S. healthcare system, rural Americans experience significantly poorer clinical outcomes. This trend is rapidly accelerating as rural hospitals close and physician shortages increase, exacerbating health disparities. In fact, Americans living in rural communities suffer a mortality rate 23 percent higher than those in urban communities, in part because of the lack of access to quality care.

Homeward supports Medicare-eligible beneficiaries by partnering with health plans, providers, and communities to align incentives – taking full financial accountability for clinical outcomes and the total cost of care across rural counties.

Homeward is co-founded by a leadership team that defined and delivered Livongo’s products, and backed most recently by a $50 million series B co-led by Arch Ventures and Human Capital, with participation from General Catalyst for a total of $70 million in funding. With this leadership team and funding, Homeward is committed to bringing high-quality healthcare to rural communities in need.

The Opportunity

We’re hiring a high agency Product Lead to lead mission-critical initiatives at the intersection of technology, AI, care delivery, and operations. You’ll own the roadmap and execution for products that directly shape how care is delivered to some of the most underserved communities in the country.

What you’ll do

  • Drive product strategy and execution for a portfolio of AI-driven healthcare products serving rural communities.

  • Operate with high agency and end-to-end ownership — from shaping vision to shipping, iterating, and scaling products that drive business critical outcomes.

  • Lead cross-functional teams across Engineering, Data, Operations, Clinical, and Design to build and scale impactful products.

  • Navigate highly operationally complex environments and develop tools that support clinicians, field teams, and patients.

  • Use AI/ML and decision support tools to enable scalable, personalized, and proactive care delivery.

  • Influence critical decisions around build/buy/partner strategies for internal tools and external solutions.

What You Bring

  • 7+ years of product management experience, ideally with experience in complex operational environments like logistics, marketplaces, care delivery, or fintech ops.

  • A strong product sense, deep curiosity, and a “run toward the fire” mentality — you thrive in ambiguity and love solving hard problems.

  • Proven track record of delivering products from zero to one, scaling existing platforms, and driving measurable outcomes.

  • Experience leading cross-functional initiatives across technical and non-technical stakeholders.

  • High comfort with data, metrics, and experimentation — you’re fluent in talking pipelines, KPIs, and A/B testing.

  • Clear, concise communicator — especially when translating complex ideas to different audiences.

  • A bias toward action, outcomes, and learning — not org charts.

Nice to Have

  • Experience with AI/ML-powered product development

  • Familiarity with healthcare, but not required — some of our strongest team members have come from outside the industry

Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the total compensation package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
