We’re Doccla, the virtual ward company. We provide patients and clinicians with the transformative power of remote patient monitoring.

Our existing customers are Hospitals, Community Services, and NHS Commissioners that we partner with to deliver tech enabled virtual wards. With our technology, logistics, and support patients can be discharged from a hospital ward to a virtual one and recover in the comfort of their own homes. Patients love our care pathways and health services become more efficient; it’s a win-win.

The purpose of this role is to help people develop the knowledge, skills, and confidence to manage their own health and wellbeing and support them to stay well. You will deliver effective health coaching sessions using a holistic and person-centred approach that helps people focus on what matters to them. The aim is to empower people to take control of their own wellbeing by building their confidence, motivating them to set goals and creating personalised action plans.

Coaches will use health coaching, motivational interviewing and behaviour change skills to empower people to make positive lifestyle changes and provide support to ensure they complete evidence based interventions such as vaccinations, regular exercise, stopping smoking, achieving/maintaining a healthy weight and completion of pulmonary rehabilitation.

The role will take a holistic approach that helps increase people’s ability to access community support and self-management education and connect to Social prescribing and other support services available. Health Coaches will work alongside clinical colleagues in the health hub to provide a joined-up approach, maximise patient experience and reduce escalations in care.

  • Work flexibly to assist with all aspects of establishing the Health Hub and onboarding patients to the service

  • Undertake patient assessments to establish their personalised Winter Health Plan

  • Providing Health Coaching support to optimise patients with long term conditions including ensuring evidence based care reaches them

  • Collect and record outcome, monitoring and impact data to contribute to the evaluation of the service.

  • Gather client feedback to evaluate the quality of the service and to inform service changes and developments
  • Forge strong relationships across all Hub partners in the NHS and beyond, helping to build a map of all local support services patients can access
  • Actively develop skills across the Health coaching portfolio on an ongoing basis

The post holder will be a highly motivated, proactive individual. They must have exceptional communication skills and be comfortable working in a flexible start up environment.

Qualifications/ Experience:

  • Have experience working with the public
  • Experience with remote patient monitoring technologies and practices is an advantage.
  • Have exceptional communication skills and be adept at building relationships
  • Be self-motivated and a problem solver
  • Ability to work independently and in a team environment
  • Commitment to the values of being patient-centred, client-focused, responsive, and trusted

Experience in the healthcare industry is preferred, but not required. Please also share with us if you have prior experience of working in an entrepreneurial environment.


Training will be provided in:

  • Understanding remote monitoring
  • Knowledge of the main long term conditions and their effective management
  • Smoking cessation (such that you will be a trained level 1 advisor)

Clinical Hub manager

The role is hybrid - based mainly at home but a degree of travel is required, including meeting clinical stakeholders in person and attending Hub Team meetings and events.
