A bit about this role:
Community Case Managers are key members of Devoted’s comprehensive care teams. As a Community Case Manager, you will serve our members as an advocate - working with members and their caregivers to identify and reduce non-clinical barriers to care. You will develop personalized plans to get members the care they need to sustain a certain quality of life and support them to achieve this plan. You will learn a lot about the community environment and how it supports (or doesn’t support) the members’ needs. Based on your expertise, you will use social services and community programs to support our members in the best way possible.
Your Responsibilities and Impact will include:
Establish trust and build strong relationships with our members
Create personalized care plans for members tailored to members’ needs
Identify and catalog available community or national resources (i.e., transportation, financial, food, medication discounts, support networks, behavioral health, etc.)
Connect members to community services and follow up to ensure those services were received
Help members (especially low income members) get the resources/care they need by helping them apply for eligible government benefits and programs
Work with family members/caregivers to help them in understanding the member’s needs and how they can be supportive
Conduct calls to members
Problem solving to break down barriers
Coordinate closely with other Devoted teams like Clinical Guides, Devoted Medical Group, Pharmacy, Telesales, and Member Service Guides
Form strong relationships with local community-based organizations and PCP offices to aid with coordination between member, Devoted, and providers (both community and health-related)
Be present, reliable and timely - for our members, for your team
Required skills and experience:
You have a minimum of 2 years doing case management or care coordination
You are able to support either Mountain or Pacific time zones (Community Guide’s hours of operation are 8:30am - 8pm eastern)
Health insurance (preferably Medicare Advantage) and/or Healthcare experience
Strong verbal and written communication skills in English; you can articulate and break down complex information
The ability to comfortably multi task: you’ll be listening, talking, evaluating and writing notes all at the same time
Ability to understand and meet performance metrics
Proficient in technology; Embrace, crave, and be comfortable with new and ever-changing technologies
You’ve had proven success in building relationships
You’re a great team member with a can-do attitude; you’re self-reflective
Ability to work in a start-up, fast-paced environment - which includes the ability to work outside your job description in some instances, including the potential for taking inbound calls
Desired skills and experience:
Advanced Degree in Social Work (i.e. LSW, LCSW, LMSW, MSW) or Advanced Degree in mental health/psychology preferred
Bilingual in English and Spanish or Haitian-Creole preferred
Experience working with seniors

Salary Range: $56,000-$80,000 / year
The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
