A bit about this role:
The Complex Care Department is a highly collaborative, multidisciplinary team of clinicians, case managers, and social workers united by our mission to build a better care delivery system for Devoted’s most complex members.
This position, Social Work Case Manager, provides whole-person clinical, social, and logistical support for patients with complex chronic conditions, frail elders, and patients at high risk of hospital admissions.
You will serve our patients as an advocate, coach, and connector, working to identify and eliminate socioeconomic and psychosocial barriers to health and self-efficacy.
You will be at the heart of our integrated care model, helping members with the highest needs navigate a complex healthcare system.
Required skills and experience:
Master’s degree in social work and active license with the willingness to gain additional state licenses as identified by program need
Minimum of 3 years working in medical social work, case management, or care coordination
Embrace, crave and be comfortable with new and ever-changing technologies
Self-starter with ability to succeed in a work-from-home environment
The ability to comfortably multi task: you’ll be listening, talking, evaluating and writing notes all at the same time
Strong interpersonal and communication skills
Comfortable working with patients over video and phone
A passion for making healthcare better for patients, solving complex problems, and supporting the delivery of healthcare that we would want for our own family members
Desired skills and experience:
Bilingual fluency (written and spoken) in Spanish, Creole, or Vietnamese.
Experience with the Medicare Advantage population and health insurance systems
Responsibilities and Impact will include:
Interdisciplinary Collaboration: Work seamlessly with clinicians, case managers, and community health workers to ensure cohesive, patient-centered care.
Psychosocial Screening & Planning: Conduct comprehensive social needs screenings and develop individualized care plans that align community, clinical, and financial resources.
Complex Family Support: Assist in navigating complex family/support issues, recognizing caregiver burnout, and facilitating access to needed emotional and practical resources.
Advanced Care Planning: Facilitate sensitive discussions regarding advanced care planning, end-of-life planning, and palliative or hospice care transitions when appropriate.
Behavioral Health Coordination: Assess behavioral health needs and coordinate care with the Devoted Medical Behavioral Health Team.
Regulatory Compliance: Uphold compliance standards, including investigating and reporting concerns for patient abuse, neglect, or exploitation.
Salary range: $70,000 - $85,000 annually

The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
