Job Description

A bit more about this role:

We want to help members navigate the healthcare system in order to get them the care they need to be the healthiest version of themselves. The Population Health Team supports members who require preventative health screenings. As a Population Health Care Coordinator, you’ll serve as an advocate for these members, coordinating care and ensuring they have the necessary resources and support to achieve better health outcomes, primarily around osteoporosis. Our ideal Population Health Care Coordinator is caring, compassionate, solution-oriented, and enthusiastic about providing an outstanding experience for Devoted Medical’s members.

Responsibilities will include:

  • Call members to check in after a fracture to see how they are doing and offer support to schedule a bone density scan to check for osteoporosis

  • Coordinate with provider offices, home health agencies and DME providers to ensure members get what they need

  • Work hand in hand with nurse practitioners and other members of the Population Health Team to address member needs towards team based goals. This may include interdisciplinary huddles and clinical rounds.

  • Provide operational and administrative support to the Osteoporosis clinical program

  • Participate in and assist with pilot programs

  • Coordinate with medical providers and facilities to access necessary records or schedule screenings

  • Follow escalation processes to clinical team to ensure members get needed evaluation and care

  • Other duties as assigned

  • You are comfortable performing care coordination for a predominantly elderly Medicare population.

  • You are able to ease the stress and anxiety of members during difficult times, quickly building trust and rapport.

  • You have a natural ability to successfully communicate and interact with different stakeholders in a member’s life from doctors to pharmacies to nurses to caregivers.

  • You are comfortable working with members over the phone.

  • You are an entrepreneurial self-starter who enjoys taking the initiative but is also a great team player willing to work hard to achieve a common aim.

  • You are comfortable engaging and learning new technologies including electronic health records and different computer platforms, operating systems and programs (Google Chrome, Google Sheets).

  • You have a passion for supporting the delivery of healthcare that we would want for our own family.

  • You're excited about being part of a program that is designing and implementing new process changes - frequent change excites you instead of making you feel stressed.

Desired skills and experience:

  • Minimum educational experience: high school diploma or GED equivalent

  • 2+ years in healthcare setting, managed care, home health, community health highly desired

  • Associates degree in healthcare related field preferred

  • Fully bilingual (English/Spanish). (Preferred)

Salary range: $20.00 - $32.00 hourly

The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
