Job Description:

A requirement for this role is experience working in an Emergency Room (ER), with a strong preference for candidates who are licensed in multiple states. The minimum requirement is to be licensed in at least one of the following states: Florida, Illinois, Indiana, Massachusetts, New York, North Carolina, and/or Ohio


The Virtual Urgent Care (VUC) Advanced Practice Physician (APP) provides care to Cityblock members as part of our innovative care model. They work on the Mobile Integrated Care team which operates 24/7 virtual urgent care and can deploy Community Care Clinicians (typically paramedic, EMT, RN, or MA trained) to members’ homes in response to urgent member clinical and behavioral needs.

The VUC APP, together with our team of virtual emergency medicine physicians, develops treatment plans and addresses acute needs via phone or video, and is able to deploy our Community Care Clinician ED at Home field team to implement assessment and treatment at the member’s home. They collaborate with Physicians during virtual visits, working closely with the Community Care Clinicians to plan next steps and close the loop with the member’s primary care team.

This role operates in rotating shifts including days, evenings, weekends, and holidays.

Responsibilities:

  • Delivers acute care for a complex population of members in a collaborative, interdisciplinary team model

  • Effectively partners and collaborates on clinical care within interdisciplinary team, ensuring highest quality of care

  • Ensures members receive appropriate care for acute and chronic diseases

  • Collaborates effectively with other Cityblock providers and longitudinal care teams

  • Prevents unnecessary emergency room visits

  • Timely documentation and loop closure

  • Actively participates in Virtual Urgent Care and ED at Home model development, redesign, and innovation

  • Works full-time with flexibility for 8-12 hour shifts, including some weekend day and holiday day call coverage

Work Experience:

  • 3+ years of experience as an Emergency Medicine Physician Assistant or Nurse Practitioner in a high-volume emergency department or urgent care. Strong preference given to those with primary care experience as well.

  • Experience in acute clinical care, managed care or value-based care operations

  • Experience providing comprehensive care management, chronic disease management, urgent home-based and community-based urgent care visits

  • Experience liaising with other providers around medical care, behavioral health, long term service and support needs, and the provision of palliative care

  • Extensive experience working in an ED or Urgent Care setting

  • Experience giving medical commands to mobile units (EMTs/Paramedics)

Education:

  • Bachelor's degree

The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:

$111,339.00 - $150,000 Annually
