About our Team:

We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.

About the Role:

As an Outreach Specialist, you are responsible for locating and connecting with pregnant and postpartum members who would benefit from the Cityblock care model, and engaging with them around their needs. This initial series of conversations with members begin the intake process, and are the stepping-off point from which a member is connected to a Perinatal Community Health Partner to develop a long-term relationship that includes but is not limited to doula support. Until this connection is fully made, you are the member’s point person, available and accessible to assist with whatever may be needed, to troubleshoot, to educate, and to be a listening ear. You work alongside, and are supported by, each member’s care team.

  • Engage and foster trust quickly with members, acting as the face of Cityblock and the key point-of-contact to assist members prior to full enrollment, or that of their newborns
  • Provide telephonic, written, digital and face to face outreach and engagement services for each eligible member. This includes member events, group welcome events and community outreach events.
  • Leverage available data from Commons to learn more about potential members and inform outreach processes.
  • Complete intake and consent process for eligible members, and seamlessly hand off to the Perinatal Community Health Partner upon full enrollment, building a deep understanding of our programs in order to offer members immediate value
  • Meet with members in their homes, neighborhoods, and the Cityblock health hub to achieve individual and team metrics and shift outcomes.
  • Go above and beyond to connect with members and their supports in a non-judgmental, respectful and empathic manner, to meet needs, and to provide feedback to our system as a whole as we strive to do better every day.
  • Work daily within Commons, our care management platform, to collect data, organize information, track tasks, and communicate with staff, patients, and family members.
  • Provide feedback to the product development team, and, over time, becoming part of a super-user group to assist in onboarding and supporting others.

This role is expected to be:

  • Member-facing
  • Based both in visits to member homes (25%) and remote work (75%)

Requirements for the Role:

  • 0-2 years of experience
  • Unrestricted Driver's License and Personal Vehicle
  • Prior doula training or interest in doula certification preferred, not required
