Role Overview
We are seeking a Nurse Manager to launch, build and lead our RN Case Management function. This is a unique opportunity for a strong operator who will initially manage cases, set standards, design workflows, and then grow into developing Nurse Team Leads and scaling the organization.
You will define and shape case management at Chamber—building teams, establishing evidence-based workflows, embedding NCQA-compliant practices, and designing an operating system that enables consistent, high-quality care at scale.
Key Responsibilities:
Build the Function
• Stand up RN case management workflows across core programs (e.g., transitions of care, longitudinal care management)
• Personally manage a panel of patients to set the clinical and operational standard
• Design care pathways, documentation practices, and escalation protocols
• Identify early gaps in care delivery, staffing, and workflows and solve them in real time
Scale the Team
• Transition from individual contributor to team leader as the organization grows
• Hire, onboard, and develop Medical Assistants, Nurse Team Leads
• Create clear roles, responsibilities, and capacity models across teams
• Build training and onboarding programs that ensure consistency and quality
Drive Performance
• Set and monitor performance metrics (e.g., engagement, LOS, readmissions, care progression)
• Use data to drive continuous improvement across teams
Clinical Oversight & Escalation
• Serve as an escalation point for complex clinical and operational cases
• Model strong clinical judgment and reinforce best practices across the team
Quality, Compliance & Systems Building
• Embed NCQA-compliant workflows and documentation into daily operations
• Build scalable systems and feedback loops that ensure quality and efficiency
• Partner cross-functionally to integrate workflows with product, data, and payer requirements
What Success Looks Like:
Fully built RN case management function, with robust workflows and high-performing teams.
Nurse Team Leads who independently manage high-functioning teams with consistency.
Measurable improvement in care progression, care transitions, and patient outcomes.

Scalable systems supporting rapid growth without sacrificing quality
Requirements:
-
RN license required; BSN preferred
Compact Nursing License or licensure to practice in DC & Maryland required
ACM, CCM, or CMAC certification preferred
5–8+ years of case management or care coordination nursing experience
Experience building or scaling teams in a startup or fast-growth environment
Track record of transitioning from individual contributor to people manager
Experience in value-based care, population health, or managed care preferred

Location: This is a remote role but you must be based in the United States.
