Role Overview

We’re looking for a Sr. Director of Care Operations & Transformation to build and lead Chamber’s care model — the operational core of how members experience our programs. You will recruit and develop care teams who make outbound calls, receive inbound support requests, and intervene on behalf of patients when needed — all designed to deliver better outcomes in a value-based cardiology model.

This role is hands-on — you’ll design the workflows, implement the technology, and drive performance. And you will be a champion for AI-enabled care delivery: shifting repetitive tasks to automation wherever safe and appropriate, so human teams can deliver rapport, clinical insight and real patient impact.

Key Responsibilities

Build & Lead Next-Generation Care Teams

  • Recruit, train, and scale care teams that proactively engage patients through outbound and inbound outreach.

  • Drive excellence in communication, documentation, and value-based outcomes.

  • Develop workflows for triage, care coordination, education, and escalation.

Operationalize AI-Enabled Care Delivery

  • Lead the adoption of AI-driven outreach, documentation, quality monitoring, and decision support.

  • Ensure care teams are working at top-of-license, shifting repetitive tasks to automation.

  • Partner with Product and Data teams to deploy, test, and scale automation that improves efficiency, accuracy, and consistency.

Build Systems That Scale

  • Own day-to-day care operations: performance dashboards, staffing models, QA programs, and capacity planning.

  • Ensure delivery against contractual SLAs and key payer metrics.

  • Translate payer requirements into clear operational workflows and accountability structures.

What You’ll Achieve in Your First 90 Days

  • Stand up care teams with the right structure, capacity, and leadership coverage for live operations.

  • Launch operating standards for outreach, documentation, and quality.

  • Activate live care programs integrated with Chamber’s platform.

  • Implement first-wave AI functionality into frontline workflows to demonstrate efficiency gains.

  • Lead an exec-level performance readout on engagement, capacity, and scalability readiness.

Requirements

  • 8–15+ years in healthcare operations (population health, managed care, care coordination, or VBC).

  • Demonstrated success as tech-forward operator, excited to deploy automation and AI into care delivery.

  • Proven success building and scaling patient-facing operational teams.

  • Operational experience translating contracts into repeatable workflows with measurable outcomes.

  • Strong analytical and systems-thinking mindset — fluent in metrics and continuous improvement.

  • Excellent communicator who influences cross-functionally with clinical, product, and executive partners.

  • Passionate about transforming heart health for patients and providers.
