Role Overview
We’re looking for a Director of Population Health to uncover opportunities through data and turn insights into measurable clinical action. This role sits at the intersection of medical economics, analytics, and operations — identifying gaps in care, cost, and outcomes, then working hand-in-hand with the Medical Director and care teams to operationalize solutions.
The ideal candidate brings strong analytical rigor, fluency in value-based care models, and the ability to translate data into execution. You’ll connect the dots between payer contracts, clinical workflows, and financial performance to help Chamber deliver better outcomes at lower cost.
Key Responsibilities
Data-Driven Insight & Medical Economics
Lead the development of population health analytics and dashboards that identify opportunities in cost, quality, and utilization.
Conduct analyses to pinpoint care gaps, high-cost drivers, and variation across populations and providers.
Translate complex data into actionable strategies that guide clinical and operational priorities.
Partner with Finance and the Medical Director to model ROI for interventions and measure realized impact.
Operational Execution & Clinical Integration
Work closely with the Medical Director to design and implement programs that address identified gaps.
Translate payer and risk contract goals into workflows that drive engagement and performance.
Coordinate between analytics, care teams, and product to ensure insights become embedded in day-to-day practice.
Define and track KPIs for program execution from enrollment and engagement to clinical outcomes and total cost of care.
Cross-Functional Collaboration & Strategy
Partner with Product and Data teams to enhance tools that surface actionable insights for care teams.
Serve as a bridge between payer analytics and clinical, ensuring alignment with performance targets.
Collaborate with leadership to prioritize interventions that deliver both clinical and financial impact.
Prepare performance narratives for payer, board, and internal reporting.
What You’ll Achieve in Your First 90 Days
Establish population health analytics dashboards highlighting cost, quality, and engagement opportunities.
Identify the top 3–5 care or cost gaps across Chamber’s active populations and develop plans with the Medical Director to close them.
Define the framework for measuring ROI and impact of clinical programs.
Deliver an ELT-level report on early population health insights and interventions in motion.
Requirements
5+ years in population health, medical economics, managed care, or healthcare analytics.
Strong understanding of claims data, cost/utilization modeling, and quality measurement frameworks (e.g., HEDIS, Stars, MLR).
Proven ability to translate data insights into operational programs.
Experience partnering with clinicians and care teams to drive performance.
Excellent communicator who can distill complexity into clarity for diverse stakeholders.
Passion for transforming cardiovascular outcomes through technology and systems thinking.
