Cadence Health team seeks an experienced Registered Nurse (RN) Manager to lead and scale multiple RN and LPN teams responsible for triage and alert workflows for patients with primary care needs, with a strong focus on hypertension (HTN), diabetes (DM), and congestive heart failure (CHF).

This role is ideal for a nursing leader who has managed virtual care or centralized triage operations and is passionate about building scalable systems, enhancing workflows, and driving high-quality outcomes for patients with chronic conditions.

WHAT YOU'LL DO:

  • Lead and mentor RN and LPN teams supporting triage and alert management.

  • Provide coaching, feedback, and development to drive clinical excellence.

  • Oversee staffing, scheduling, and workload to ensure efficient, high-quality care.

  • Foster a collaborative, patient-centered culture grounded in accountability and empathy.

WHAT YOU’LL NEED:

  • Active RN license in good standing (multi-state license preferred).

  • 5+ years of clinical RN experience, with at least 2 years in a leadership or management role within triage, care coordination, or virtual care.

  • Strong foundation in primary care and chronic disease management, especially HTN, DM, and CHF.

  • Experience managing remote RN and LPN teams in a high-volume environment.

  • Proven success scaling clinical operations within a growth-stage or startup environment.

  • Proficiency with EHR/eRecord systems and data analysis tools for performance monitoring.

  • Excellent leadership, communication, and problem-solving skills.

  • Skilled in nursing processes, documentation, and clinical decision-making.

  • Technical fluency; able to work seamlessly across platforms including Notion, Athena, EPIC, Zendesk, and G Suite.
