Clinical Navigation Trainer
Cadence Solutions
Across the United States, 6 in 10 adults – or 133 million Americans, live with one or more chronic conditions. Chronic disease is today’s leading cause of death and disability in the US and the leading driver of the nation’s $4.1 trillion in annual healthcare costs. Patients who live with chronic conditions require far more touch points than our primary care physicians have time to deliver and the result is countless health emergencies and costly ER visits that could be prevented. Workforce shortages and lack of technologies make it difficult to give patients the attention they need.
At Cadence, our mission is to deliver life-changing care to over one million patients living with chronic disease by the end of the decade. We mitigate the impact of chronic disease by using technology and a world-class clinical team to remotely monitor, manage, and support patients at home. Together with our growing network of health system partners, we deliver guideline-directed care to tens of thousands of patients today while producing best-in-class clinical outcomes.
The Cadence Health team is seeking a Clinical Navigation Trainer to join our Care Delivery team. This role is dedicated to developing, implementing, and evolving high-quality onboarding and ongoing education programs for our Clinical Navigator team, which includes Licensed Practical Nurses, Medical Assistants, and Health Coaches. The ideal candidate will be passionate about education, clinical excellence, and improving patient care through structured training and support resources.
WHAT YOU’LL DO:

- Collaborate with clinical leadership and subject matter experts to develop ongoing training for current team members.
- Continuously evaluate and revise training materials and curricula based on learner feedback, performance outcomes, compliance updates, and evolving clinical workflows.
- Support the creation, documentation, and ongoing refinement of Standard Operating Procedures (SOPs), care pathways, and workflow guides for the Clinical Navigator team.
- Lead the facilitation of virtual training sessions, skills refreshers, and continuing education initiatives.
- Develop training assessments and post-training evaluations to ensure comprehension and effectiveness.
- Serve as a partner to Clinical Navigator supervisors, aligning L&D efforts with team OKRs, quality improvement initiatives, and performance trends.
- Stay current with industry best practices in adult learning and clinical training, applying innovations to enhance training impact.
- Maintain a structured library of educational content, SOPs, and supporting materials for easy access and version control.
- Collaborate cross-functionally with Clinical, Product, Quality, and Operations teams to ensure training content reflects the latest tools, policies, and workflows.
- You'll be expected to collaborate and work cross-functionally with our Learning & Development (L&D) Care Delivery team.
WHAT YOU’LL NEED:
- 5+ years of experience in healthcare, preferably with exposure to clinical training, patient navigation, or chronic condition management (e.g., diabetes, hypertension, CHF).
- 5+ years of experience designing and facilitating clinical or healthcare education programs.
- Strong understanding of the clinical navigator role, including workflows performed by MAs, LPNs, and Health Coaches.
- Experience developing SOPs and training materials in a fast-paced, evolving environment.
- Exceptional written and verbal communication skills, with a strong ability to translate complex clinical information into clear and accessible training.
