Work Schedule: Full-time, hybrid (on-site at local senior facilities work and remote administrative work)

This is done by partnering with senior living communities to coordinate and manage clinical gaps for residents to improve these facilities’ core business drivers. Job Overview:

Savoy Life is seeking a dedicated Resident Care Coordinator to provide clinical and operations support within senior living communities in Arizona. This is a hybrid role where clinical care and coordination will be conducted on-site at senior living facilities, while administrative tasks and other care coordination can be done remotely from home. The Resident Care Coordinator will work closely with an assigned Savoy Life RN as well as on-site senior living facility teams to improve resident outcomes.

Key Responsibilities:

  • Clinical Support
    • Support the RN and Advance Practice Provider (APP) by coordinating patient care to drive patient outcomes including managing orders/referrals, facilitating telehealth appointments, preparing patient charts, and more
    • Manage communications and foster positive relationships with patients as well as others involved including caregivers, families, primary care providers, specialists, pharmacies, and senior living facility staff
  • Resident Engagement
    • Help drive engagement with eligible residents through education on Savoy’s offering, outreach to providers and caregivers, event planning within the senior living facilities, and more
    • Maintain an understand of Savoy’s offering for patients and clearly communicate this to the various stakeholders
    • Mail communications to different individuals and groups such as patients’ families and primary care providers
    • Coordinate with new patients and caregivers to get consent forms signed
  • Back Office Coordination
    • Schedule patients in the EHR to optimize the RN and APP’s time while coordinating with patient and caregiver needs
    • Answer inbound calls in a professional and timely manner
    • Monitor and manage inbound and outbound faxes
    • Order medical supplies and other branding materials
    • Enter data and other files in the EHR as needs arise in an organized and timely manner
    • Work with the local Savoy team to collaborate with other community resources that aid in Savoy’s goals of supporting patient care
    • Support other vendor needs that may arise at a patient or local level

Qualifications:

  • High School Diploma / GED (or higher)
  • 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
  • Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience
  • Experience in geriatric care or senior living facilities is highly desirable
  • Must possess a valid and current driver's license
  • Excellent electronic medical record (EMR) documentation skills and general comfort leveraging various technologies to support day to day work

Salary Range: $45,000 – $60,000. This represents the typical salary range for this position based on experience, skills, and other factors.
